What happened?
The FDA approved Eli Lilly’s once-daily oral GLP-1 pill Foundayo (orforglipron) for adults with obesity or overweight with weight-related complications on 1 April 2026, shipping began via LillyDirect on 6 April and retail pharmacy availability followed shortly after. Foundayo is a once-daily small-molecule, non-peptide GLP-1 receptor agonist that can be taken at any time without food or water restrictions — a key differentiator from Novo Nordisk’s oral Wegovy, which requires fasting conditions.
Why does it matter?
The approval brings the oral GLP-1 market to two competing products within three months, creating the first direct pill-on-pill rivalry in the fast-growing obesity drug space and putting pressure on both companies to compete on access, price, tolerability and convenience as much as efficacy. Goldman Sachs projects oral GLP-1s will capture around $22 billion (24%) of the global weight-loss market by 2030, with Foundayo expected to take a 60% share of that segment — versus 21% for Novo’s pill — largely because orforglipron requires no food or water restrictions and may prove more accessible at scale.
What’s next for Novo?
Novo Nordisk moved quickly to defend its three-month market lead, publishing a cross-trial comparison on 2 April claiming its 25 mg oral Wegovy delivered 3.2 percentage points more weight loss than Foundayo’s highest dose, with patients on Foundayo having roughly 14 times the odds of discontinuing due to gastrointestinal side effects. Lilly disputed the comparison as indirect and potentially misleading, noting that Foundayo’s no-food-restriction dosing and lower starting price ($25/month with insurance) address access barriers that Wegovy’s pill has not fully resolved.
What’s next?
Lilly has submitted orforglipron for weight management and type 2 diabetes in more than 40 countries and plans to launch immediately after each national approval. In parallel, it continues developing retatrutide, a triple hormone receptor agonist showing even greater weight loss in trials. Novo, meanwhile, is advancing amycretin in late-stage trials, suggesting the oral GLP-1 rivalry will escalate further across both efficacy and modality dimensions over the coming 12–18 months.
What are people saying?
Lilly CEO David Ricks: “Fewer than 1 in 10 people who could benefit from a GLP-1 are taking one… Foundayo can help level the playing field.” Novo Nordisk: “Not all GLP-1s are the same. Any reports claiming orforglipron is more effective than Wegovy pill for weight management are inaccurate and misleading.”
